First Care Project’s multidisciplinary team provides a full continuum of HIV services for people seeking compassionate and professional HIV/AIDS services through treatment. Our HIV care team is comprised of American Academy of HIV Medicine-certified physician assistants, registered nurses, case managers, prevention specialists, and strives to provide high-quality patient-centered care for optimal health outcomes. Our goal is to help you navigate a complex healthcare system so that you can be an active partner in managing your chronic illness.

The PHC pharmacy dispenses medicines to patients treated in our clinics. As a 340(B) pharmacy, we are able to provide patients with affordable medications. Patients can also apply for other assistance programs. Clinical pharmacy services can also help patients with chronic health conditions. A pharmacist can also be consulted for medication, glucometer, diabetes management and nutrition.
Our team of homeless support services professionals work with local homeless shelters, family and domestic violence shelters, and other nonprofit organizations to provide timely, effective, and compassionate services through a centralized intake program. PHC case managers assess each individual or family and determine the best course of action to support them.
A centralized approval process can help with housing placement, business management and unlocking income opportunities. A referral to another community service may be the next step, and PHC social workers will provide “hot referrals” to ensure the individual or family receives the best care and services from the facility.
Unitypoint Health Younker Rehabilitation Clinic
The Homeless Support Group works with a network of local landlords to provide housing to those in need. Support services for veterans and their families is a program offered by HSS. There is also a shower and toilet for those who need it.
Supportive Services for Veterans Families (SSVF) is a program funded by the Department of Veterans Affairs that aims to provide stability in low-income housing. The program is offered in Dallas, Jasper, Marshall, Polk, Story and Warren counties. SSVF provides information and business management services. Case managers help veterans and their families find housing and provide guidance on applying for VA and other public benefits.
A veteran is defined as a person who served in the military, naval, or air services and was discharged or discharged under conditions other than honorable.
