With ever-increasing costs, many providers are unwilling or unable to serve small populations and other challenges, said Dr.

“Some patients cannot speak English, so an interpreter is always needed. “There are also a lot of people dealing with mental health issues and drug and alcohol abuse,” Garner said.

In 2018, about 47 percent of dentists covered by Medicaid provided dental services to children age 12 and younger, according to I-Smile data released by the Iowa Department of Health . That’s a drop from five years ago.

It’s more difficult for seniors on Medicaid. Because they often require expensive procedures, few dentists offer public health insurance for seniors. Also, annual financial limits mean that only dental emergencies are covered.

“It seems like you’re sending them another problem until their insurance comes back next year. I think that’s unfortunate,” Garner said.

Community service is a personal choice, Dr. Garner says, even if it doesn’t mean a lot of money.

“There aren’t many places where I can give them respect and dignity and love,” Garner said.
